Clinical evaluation of oculo-cardiac reflex (part 1)

Abstract
1) In this study 112 healthy children scheduled for surgical correction of strabismus were investigated. Their ages ranged from 4 years to 16 years. Preoperative medications were abolished. Induction and maintenance of anesthesia were standardized. Halothane, nitrous oxide and oxygen were used for the general endotracheal anesthesia.
2) The children were divided into two groups; Group A was non-atropinized children to prevent the bradycardia or arrhythmia, Group B was atropinepretreated group. Atropine(0.01mg/kg) was administered intravenously less than 30 minutes before the operation.
3) An oculocardiac reflex was considered elicited if the heart rate slowed by 15 or more from preoperative levels or if arrhythmia occurred during traction of the extraocular muscle.
4) The incidence of the oculocardiac reflex in the non-atropine group revealed 68.8% and in the atropine group revealed 35.3%.
5) The most recommendable method to prevent the bradycardia, arrhythmia and cardiac arrest was continuous monitoring and close observation of the patient. And it is important too to keep adequate ventilation to avoid hypoxia during surgical correction of strabismus.
